BridgesHome provides the skilled services of nurses, physical, speach, and occupational therapists under the direction of a physician's order. In addition, personal care from a home health aide and assistance with psychosocial issues by a medical social worker are available. A typical scenario might be a patient who had surgery and requires on-going skilled treatments such as wound care or I.V. medications. They may have nutritional needs or be newly diagnosed with diabetes and need to learn how to administer their insulin, manage their diet and understand the disease. The goal is to teach the patient and caregiver to become independent in their care and achieve the best possible outcomes.

Another example may be a patient who has had a stroke and needs multiple rehabilitation therapies. We then provide an interdisciplinary care plan to coordinate all the services necessary for recovery. The medical social worker can also link patients to the appropriate community resources for assistance.
