Founded in January 1999, Heart to Heart Home Health Care, Inc. is a subsidiary of American Healthcare Centers, and an extension of the services offered to the local community by Bob and Phyllis Leatherman. Heart to Heart Home Health Care, Inc. is a full-service home healthcare agency certified by Medicare, Medicaid, and two Passport agencies. We provide long-term in-home medical care, including nursing and home health aides, skilled nursing, and physical and occupational therapy. The majority of our focus is on long-term care in-home which enables people to remain in their own homes rather than going to nursing homes.

We're a privately owned, personalized health care company, and we strive to build strong, personal relationships with our patients and their families. Even though I'm the Administrator, I review each case personally and get to know every patient. We speak to them on the phone frequently and make sure they feel comfortable bringing their needs and concerns to our attention.

Our aides are in the home from one to eight hours a day, helping to perform a wide variety of tasks. We assist our patients with personal care, light housekeeping duties, accompany them to physician's appointments, run errands, get groceries, and prepare meals.

Heart to Heart Home Health Care, Inc. is a certified Medicaid and Medicare service provider, and we provide services under several Medicaid waivers. The Passport Waiver enables us to provide services to those who are over 62 years of age, and the Ohio Homecare Waiver enables us to serve those handicapped patients who are under 62 years of age. Our service area includes all of Medina County, the western Summit County communities of Barberton, Norton, and Portage Lakes, and the northern Wayne County communities of Rittman and Seville.
